Diet chart for thyroid problems
No food cures a thyroid problem; the tablet does the main work. Food matters in two ways: taking the tablet the right way, and eating a normal balanced diet that keeps weight steady.
- Underactive thyroid (hypothyroid): take the thyroxine tablet with water on an empty stomach, then wait 30 to 60 minutes before tea or breakfast.
- Keep calcium, iron and antacid tablets, and soya milk, at least 4 hours away from the thyroid tablet.
- Use iodised salt as normal, no more. Do not take extra iodine, kelp or "thyroid booster" pills.
Eat more
- Vegetables of all kinds, including cooked cabbage, cauliflower and broccoli
- Overactive and losing weight: eat enough, with an extra snack of milk, curd, nuts or egg
- Dal, chana, rajma, sprouts, paneer, curd, eggs, fish, chicken
- Whole fruit twice a day: guava, apple, orange, papaya, pear
- Whole grains: atta with chokar, bajra, jowar, dalia, oats
- Water and chaas; walking every day to keep weight and energy up
Eat less
- Underactive: rice, roti, potato and sweets in large amounts, as weight gains easily
- Overactive: tea, coffee and cola, which worsen the racing heart and shakes
- Soya: normal amounts in food are fine; keep it 4 hours away from the tablet
- Cooking oil and ghee together: 4 to 5 teaspoons per person for the whole day
- Fried snacks: samosa, pakora, namkeen, chips
- Salt: less than one teaspoon a day in all, and it should be iodised salt
Avoid
- Tea, coffee or milk with the thyroid tablet
- Kelp, seaweed and iodine drops or pills, unless a doctor prescribes them
- Overactive: energy drinks and too much caffeine
- Crash diets and fasting to lose weight
- Stopping or changing the tablet dose on your own
- Alcohol and smoking (smoking makes thyroid eye disease worse)

Common myths
- Cabbage, cauliflower and soya must be stopped.
- Cooked, in normal amounts, they are safe.
- Thyroid means you can never lose weight.
- Once the level is controlled, weight responds to diet and walking like anyone else’s.
- Extra iodine or sea salt cures thyroid.
- Iodised salt is enough. Extra iodine can make thyroid problems worse.
- The thyroxine tablet can be stopped once TSH is normal.
- Normal TSH means the dose is right. Most people need thyroxine for life.
Dr. Arya’s note for you
Overactive thyroid (hyperthyroid): go to a doctor at once if you have a very fast or irregular heartbeat, high fever, confusion, or on carbimazole a sore throat with fever.
This is general advice for adults. Pregnancy changes thyroid doses and needs a check at once. If you also have diabetes or heart disease, follow that chart and take the thyroid tablet as shown here. When in doubt, ask Dr. Arya.
